Generalized anxiety disorder (GAD) is characterized by persistent, excessive worry that interferes with daily functioning (eg, occupationally, socially) and occurs for at least 6 months. Treatment for GAD includes psychotherapy (eg, cognitive-behavioral therapy) and/or pharmacotherapy (eg, selective serotonin reuptake inhibitors [SSRIs] ).
SSRIs (eg, fluoxetine, sertraline) are prescribed for long-term management of anxiety in clients with GAD that work by increasing the available levels of serotonin, a mood-enhancing neurotransmitter. However, clients may not see a full therapeutic effect until 4-6 weeks after starting SSRIs (Option 2).
(Option 1) Lithium is a mood stabilizer most often used for the treatment of bipolar disorder. Lithium has a narrow therapeutic range (0.8-1.2 mEq/L [0.8-1.2 mmol/L]); therefore, serum lithium levels should be monitored carefully throughout treatment.
(Option 3) Aripiprazole is an atypical antipsychotic used for the treatment of irritability associated with autism spectrum disorder, schizophrenia, and bipolar disorder. Aripiprazole has a more favorable safety profile (eg, less metabolic effects, lower potential for prolactin release) than typical antipsychotics.
(Option 4) Methylphenidate is a central nervous system stimulant used for the treatment of attention deficit hyperactivity disorder. Methylphenidate improves focus and attention by increasing levels of certain neurotransmitters (eg, dopamine, norepinephrine) in the brain.
