How to Answer NCLEX Fundamentals Questions

Success in fundamentals comes from understanding priorities, safety, delegation, and patient assessment. Learn the patterns behind the questions, and unfamiliar scenarios become easier to solve.

Identify the First Course of Action
When multiple answers seem correct, focus on what should happen first. Assessment, intervention, notification, and documentation each have a place, but not in the same order.
Sort Safety from
Comfort
The safest answer is often the correct answer. Falls, restraints, infection precautions, and patient identification frequently outweigh convenience or comfort.
Match Interventions to Problems
Know which actions solve which problems: repositioning for pressure injury prevention, hand hygiene for infection control, ambulation for circulation, and oral care for aspiration prevention.
Know Delegation Boundaries
Delegation questions are easier when you know the limits of each role. RNs assess and evaluate, LPNs reinforce and monitor, and UAPs assist with routine care.
Recognize Signs
